Problem

When a tool is blocked due to agent permissions, it shows as running (◇) but never completes. Users don't know if it succeeded, failed, or was denied.

Example: PM agent's ls -la command was prevented by permissions but shows:

◇ bash  ls -la /Users/janni/git/ratcher | head -20

Desired State

Permission-denied tools should show with red strikethrough:

✗ bash  ̶l̶s̶ ̶-̶l̶a̶ ̶/̶U̶s̶e̶r̶s̶/̶.̶.̶.̶  (denied)

Or simpler with red color and icon:

✗ bash  ls -la /Users/... (permission denied)

Requirements

• Detect when tool was blocked by permission system
• Render with red ✗ icon
• Show "(denied)" or "(permission denied)" suffix
• Optionally use strikethrough text if terminal supports it

• Need to check how permission denials are reported in tool events

Acceptance Criteria

• Permission-denied tools render distinctly from errors
• Red ✗ icon indicates denial
• Text indicates it was a permission issue, not a crash
