Restore activation.entry from its original native history snapshot after same-document history changes, including initial replace activations with no from-entry. Preserve entry identifiers, keys, history/navigation state and author-shadow isolation across runtime replacement.

Validation on base
aec7fa782988: cargo fmt --all; cargo clippy --workspace --all-targets --all-features -- -D warnings; cargo nextest run --no-fail-fast. All passed (20,519 passed, 18 existing skips; 32 build jobs and test threads, no automatic retries). Focused regressions also passed.
