Lock the inode and open-file lists, and make sure concurrent Minix cache lookups don't load the same inode twice. Keep the locks held through file release and inode cleanup.
Also make inode reference counts atomic and lock Minix inode block updates, so concurrent writes don't overwrite each other's changes.
Fixes #62.
This might need a bit more testing before merging. I only tested it on baseline SWEB - we should try this on a full SWEB with many features and filesystem interactions.
