I noticed that the extension was no longer active. I wasn't sure why this happened, but going into Safari prefs and enabling it did not re-enabled the extension, even though the extension checkbox was ticked.
In order to re-enable the extension I had to make sure the checkbox was not ticked, then quit Safari, then attmept to re-enable. Once I did this, I got a never before seen message that said:
"Safari detected an app or service that interfered with clicking"
After accepting the message prompt(s), the extension was successfully re-enabled without the need to reinstall.
I am not sure what this is all about, but I noticed this issue in the BitWarden repo. It referenced this Apple help article.
