  1. echo-otel/v4: Echo v4 support, version 4.0.0 

    Echo v4 line of the middleware, github.com/labstack/echo-otel/v4, for otelecho
users who stay on Echo v4. Same API and telemetry as v5, with echo.Context.

- module github.com/labstack/echo-otel/v4, ScopeName = module path, Version 4.0.0
- minimal Echo v4.15.4 (same as otelecho), CI also tests the latest Echo v4
- ResolveResponseStatus mirrors Echo v4's DefaultHTTPErrorHandler; http.route
  from c.Path(); a 5xx HTTPError reports its status code as error.type;
  OnNextError with c.Error(err) for custom error handlers
- example ported to Echo v4; workflows run on the v4 branch
- a Recover middleware added after this middleware sends 500 through the error
  handler; reported as a 5xx response (error.type "500")
