Refactor (#219) * Fix broken plugin specs, prune dead plugins, lazy-load Three config blocks were silently dead: - refactor.lua declared two plugins as array entries, so lazy.nvim iterated ipairs and discarded dependencies/lazy/opts/config. Also refactoring.nvim removed its telescope extension upstream, so the load_extension call would have errored anyway. Now uses the native select_refactor() and adds async.nvim (required on 0.12). - lsp.lua passed a `handlers` table to mason-lspconfig v2, which removed that option entirely. The default handler never ran, so cmp capabilities were never applied to any server and lua_ls settings never took effect. Replaced with vim.lsp.config("*", { capabilities }) plus per-server config, matching the 0.11+ API. - livecode.lua set both `keys` and `lazy = false`; lazy registered the keymaps then deleted them again on eager load. Removed: Comment.nvim (native gc since 0.10), indentLine (unmaintained, hijacks conceallevel), telescope-smart-history (needs uninstalled sqlite.lua), FixCursorHold (upstream bug long fixed), nvim-tree (oil is the explorer in use), the copilot cmp source (copilot-cmp not installed), and cmp-path/cmp-cmdline (never registered as sources). Keymaps: gt/gu were bound outside the fugitive autocmd and shadowed the builtin next-tab and lowercase operator globally; now scoped to diff buffers. [d/]d were reversed and shadowed the 0.11 builtins; dropped. / quickfix maps were unreachable behind the window-nav maps; moved to ]q/[q. d blackhole-delete moved to D so the eight d* maps stop waiting out timeoutlen. Lazy-loading: 56 of 61 plugins loaded eagerly. Added event/cmd/keys triggers throughout and split nvim-cmp out of the lspconfig spec onto InsertEnter. Now 9 eager, startup 98ms -> 22ms. Also: ColorMyPencils() defaulted to rose-pine-moon, so every zen-mode toggle switched the colorscheme off kanagawa; it now defaults to the active scheme. telescope's git_worktree extension was never loaded, so gwt was broken. wrap_results was nested under `extensions` instead of `defaults`. after/ftplugin/lua.lua used vim.opt instead of vim.opt_local and leaked shiftwidth globally. The BufWritePre whitespace strip clobbered the search register and cursor, and mangled markdown hard line breaks. Dropped dap's DEBUG log level, made the neotest rspec container id overridable via $RSPEC_CONTAINER, moved mason to the mason-org org, and replaced deprecated vim.loop / vim.tbl_flatten / vim.highlight / diagnostic.goto_* calls. * Remove nvim-dap, pin neotest's container by name No DAP adapters were ever installed (mason-nvim-dap had an empty ensure_installed, and the mason package dir holds only LSP servers), so nvim-dap, nvim-dap-ui and mason-nvim-dap were dead weight. nvim-nio and mason.nvim stay behind: neotest and nvim-lspconfig still depend on them. Frees , -, b, B and d{r,s,w,b,S,c}. b in particular was shadowing codediff's explorer toggle. Dropped the td keymap along with it. It was a byte-identical duplicate of tr despite being labelled "Debug Nearest Test", and the strategy = "dap" it briefly carried had no Ruby adapter to talk to. neotest's rspec command now targets the container by its compose-derived name instead of a raw id, which survives a rebuild. Also dropped -t from docker exec: it fails with "cannot attach stdin to a TTY-enabled container" whenever stdin is not a terminal, and only worked because neotest's integrated strategy happens to set pty = true. Plugin count 55 -> 52.
